MPA Students - IRB and Federal Requirements
It is college policy to ensure that the rights and welfare of human subjects are adequately protected in research conducted under its auspices. In addition, federal and state laws and regulations require these protections. In order for the college to fulfill its responsibility and to comply with the law and regulations, all human subjects research conducted under college auspices must receive appropriate review and approval. ALL PAD 690 projects involving human subjects must be be approved in the IRB process. MPA Projects involving Human Subjects Research are approved at one of three levels - 2, 3, or 4. The IRB level is determined in PAD 699 or PAD 690 in consultation with Buffalo State's IRB Administrator.

Consult your 681 or 690 instructor for guidance whether you should submt an exempt or expedited application. (Full board review is unusual in MPA project research.)
EXEMPT APPLICATION PROCEDURE
SIGNATURES: Student signs as project director and the faculty MPA project supervisor signs as sponsor (exempt application). The student emails the signed IRB application and all supporting documentation to Ms. Gina Game GAMEG@buffalostate.edu with copy to the faculty sponsor. Type "IRB Application" in the subject line.
EXPEDITED APPLICATION PROCEDURE
Expedited applications require Dr. Buonanno's signature (signs under "faculty chair") in addition to signature of the project director (the MPA student) and the project supervisor (PAD 690 instructor).
The student (project director) should EMAIL the completed, SIGNED form - project director and faculty sponsor - to Dr. Buonanno for her signature, along with the full application. Dr. Buonanno will sign and return by email the application to the student/project director.
THe student/project director must email the IRB application form and all supporting documentation to GAMEG@buffalostate.edu with "IRB application" in the subject line. The student should copy both the faculty sponsor and Dr. Buonanno firstname.lastname@example.org.
Informed Consent Form - Students should use this template for the informed consent requirement of their IRB application. Informed consent is not used for mailed/web-administered surveys.
Boiler plate - a doc file for expedited application - adjust as needed
This course is completed as part of the PAD 680 course requirements. Students must attach their CITI certificate to their IRB application. In case of missplaced/lost certificate, log on to the CITI site to retrieve and print. Look for "Social and Behavioral Responsible Conduct of Research" - Completion Report - there will be a link to print and/or download your report.
